Loan Forgiveness program Position Summary Advance Community Health
is currently staffing for new Nurse Practitioners and Physician
Assistants to work our new Advance Axcess (Urgent Care) Clinic
located in Raleigh, NC. The Advance Nurse Practitioner (NP/PA) will
work under the general supervision of the Executive Medical
Director and have a work schedule of 4 x10hr days. The Provider
will evaluate and treat patients in an efficient and effective
manner when immediate care is needed. This may include completion
of referrals, consults, daily patient interaction, daily notes,
procedures, clinic notes, pre and post procedure care, prescribing
and patient care follow-ups. Will work closely with other
healthcare providers in a collaborative multidisciplinary approach
to maximize the care we provide to our patients. Essential Duties
and Responsibilities Obtains relevant health and medical history,
performs thorough physical assessment, reviews and interprets
pertinent diagnostic tests/results. Conducts preventive health
screening based on age, history and patient needs. Develops
comprehensive differential diagnoses for patients. Revises the plan
of care to meet the changing needs of the patient. Prioritizes work
and assists staff to provide prompt and efficient patient flow.
Interacts with patients and families in a respectful, courteous and
timely fashion using appropriate communication for the patient and
situation. Working closely with other healthcare providers in a
collaborative multidisciplinary approach to maximize the care we
provide to our patients Will function as an educator for
PA/NP/CNM/DO students from healthcare education programs. Will
maintain the scope of practice to be consistent with the policies,
procedures and regulatory guidelines governed by the North Carolina
Medical Board, Nursing Board and any other federal or state
regulatory agencies, as well as the policies and procedures.
Provide follow-up care to patients. Providing sound advice to
patients regarding lifestyle choices; diet, exercise and medication
use. Education and Experience Bachelor's degree from an
appropriately accredited institution and three (3) years of
experience in the program related to the area of assignment; OR an
equivalent combination of training and experience and Master's
Degree Nursing Certified by the American Association of Nurse
Practitioners or similar and appropriate state licensure. Basic
Life Support (BLS) Certification in Advanced Life Support 3-5 years
directly related experience required. Work Environment The work
