Job Description:
A company is looking for a Bilingual Spanish Telephonic Care
Coordinator.
Responsibilities:
Conduct monthly phone calls with patients, practices, pharmacies,
etc. to ensure patients' needs are met
Create and revise personalized care plans for each patient in
coordination with practices and providers
Identify social determinants of health, gaps in care, and
eligibility for assistance or referral services
Qualifications:
1-2 years healthcare experience as a Certified/Registered Medical
Assistant with active accreditation
Technology experience, including familiarity with multiple EMR/EHR
systems and Microsoft Office
Highly professional and compassionate phone demeanor with strong
critical thinking skills
Highly self-motivated, organized, and able to work
independently
Must have a home office setup with a second monitor and fast,
reliable internet (50mbps or greater)
