Information:Sign-On Bonus: $500 at 30 days, $500 at 6 months. The
Time-Type:Part timeJob Description:1. Assess each person receiving
services for their hopes, dreams, and desires and provide supports
that facilitate achievement.2. Ensure that the rights of each
person receiving services are protected and promoted at all times.
Assist people in developing advocacy skills, participating in
advocacy efforts, and utilizing personal advocates, as well as,
provide periodic relief to the caregiver.3. Serve as a resource for
individual receiving services on community agencies, services, and
supports that can meet identified needs/goals.4. Supervise,
educate, and monitor (as needed) individual receiving services in
work, home, school, or community type settings.5. Provide
one-on-one support as needed to meet the emotional, physical, and
medical needs of each person supported.6. Maintain a safe
environment for community, employees, and individual receiving
services by practicing safety procedures.7. Facilitate
person-centered, effective, positive relationships with individual
receiving services using positive approaches that promote
self-determination in all areas of life.8. Provide input and
recommendations into assessment and planning processes, and
development of the individual's plan.9. Implement person's plan
fully including but not limited to respite plan, within the
requirements of the service definition, and document as required by
the service definition and agency policy. Accurately complete all
required documentation, including but not limited to, goal
completion, actions toward outcomes, incident reports, timesheets,
etc.10. Complete daily progress notes and communication log to
assure appointments, goals, and interests are met and/or daily
documentation.11. Assist in maintaining all necessary records,
daily attendance, check sheets, production sheets, etc.12.
Substitute in-house or in the community as demands occur.13. Assist
new staff and/or current staff with orientation, mentoring, and
training.14. Attend and actively participate in meetings and
training as required. Maintain certification in all agency, state
and federal training requirements.15. Demonstrate knowledge of and
comply with all agency policies and procedures, as well as state
and federal statutes and regulations related to specific program
areas.16. Follow service definition guidelines for services being
provided.17. Perform all other duties as assigned by the
supervisor18. Driving and travel may be required.Education:High
School (Required)Certifications:North Carolina Driver License
