Person Memorial Hospital - Extended Care Unit

Leads and coordinates key strategic development and functional services to support department objectives and organizational goals. Maintains effective and responsive relationships with patients/residents, attending medical staff, and other departments.

In the execution of job duties, it is the universal expectation that all tasks are performed with a patient centered focus, while also seeking opportunities to continually improve core processes. Employee will be scheduled based on operational needs, which may include but is not limited to: holidays, extended shifts, night and/or weekend shifts, standby and/or on-call.
Leads and coordinates all functions related to the coordination of social services for all long term care residents and short stay skilled patients within the Extended Care Unit. Including but not limited to the following activates: discharge planning process, social services assessments, care plan conferences, coordinate grievances, coordination of social and ancillary services, advocate for residents/patients etc.
This role is expected to cross train in admissions coordination and will provide support and backup to the admissions coordinator as needed.

Reports to: Social Services and Admissions Supervisor

FLSA: Non-Exempt

Job Requirements:
Minimum Education :
Bachelor's degree is Preferred in Psychology, Sociology, Social Work or human services field.
Required Skills:
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
Minimum Work Experience:
Preferred - 2 years' experience in Long Term Care case management or Social Work experience.
