The PDC owns and operates more than 120 primary and specialty care clinics in 10 North Carolina counties. Triangle Heart Associates General Description - Coordinate plans for the professional developmentofthenursing staff assigned to aspecialty unit area and serve as a practitioner of specialized patient care. - Coordinate the development and implementation of educational programs designed to convey specialized knowledge and skills to nursing personnel and patients. - Coordinate with the nursing staff to identify patient care problems, to implement approaches to these problems and to evaluate the effectiveness of the approaches. - Serve as a resource person in a nursing specialty area for patients, nurses and other health team members throughout Duke University Medical Center and community agencies. - Participate in the Quality Assurance Program in Nursing for the development, implementation and evaluation of patient care. - Maintain liaison with faculty members of schools of nursing toarrange and facilitate clinical experience for nursing students; communicate with faculty members concerning students' needs and progress. - Perform other related duties incidental to the work described herein. Job Duties 1. Clinic Coordination: Coordinate with PSA and Staff Assistants to assure clinics full ofappropriate patients;obtain new patient records, organize, tab them for MD review; ensure return patients havenecessary tests/labs; surgicalclearance -send to necessary MD; schedule and reschedule makeup clinics; 2. During Clinic: Conduct pre-visit with patients if time allows and report out to provider; collaborate/coordinate with clinic staff to recheck vital signs as needed; educate patients regarding labs, procedures, medications, diseases; teeup orders for tests/procedures-caths/TEE/CV/Stress echo as needed; coordinate hospital admissions, que all refills in clinic-AEMR and review with Providers. 3. Clinic follow-up: Follow up to assure labs/procedures are completed, discuss abnormal results with provider, call patients with explanation of abnormal results, annotate normal results and forward to patients via MyChart or mail only if no MyChart account. Coordinate labs/studies with outside facilities, obtain appointments with specialists and coordinate w/pt., review all grid sheets after clinic-to ensure items were completed. 4. Triage: Manage incoming patient calls, coordinate plans with providers, and return patient calls. Manage prescription refills as directed by provider. Complete prior authorizations, patient/insurance requested forms, and coordinate referrals. Knowledge, Skills, and Abilities - Cardiology experience preferred - Ability to lead others - Strong customer service skills - Strong conflict resolution skills - Strong critical thinking skills - Desire to learn new skills - Ability to think proactively and respond professionally - Knowledge of scope of the registered nurse, licensed practical nurseand all unlicensedassistive personnel - Knowledge of and appropriate application of the nursing process - Knowledge of professional theory, practice and procedures - Ability to assess nursing needs of acute and chronically ill patientsand their families - Ability to independently seek out resources and work collaboratively - Ability to establish and maintain effective working relationships - Ability to communicateclearly with patients, families, visitors, healthcare team, physicians, administrators, leadership and others - Ability to teach patients and families in accordance with the nursingplan of care - Ability to use sensory and cognitive functions to process andprioritize information, treatment, and follow-up - Ability to use fine motor skills - Competent in BLS and/or other specialized life support requirements designated by work areaor organization - Ability to record activities, document assessments, plan of care, interventions, evaluation and re-evaluation of patient status - Ability to use the computer, proficiency with basic computer skills,and ability to learn new software programs - Ability to document and communicate pertinent information using computer and/or paper documentation tools - Ability to navigate the entity to provide clinical care for patients - Ability to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds - Ability to remain focused and organized - Working knowledge of procedures and techniques involved in administering routine andspecial treatments to ambulatory patients - Working knowledge of infection control procedures and safety precautions Education Requirements/Preferences Work requires graduation from an accredited Bachelors of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program. State of North Carolina RN licensure is also required. Experience Requirements/ Preferences Work requires three years of registered nursing (RN) experience OR AN EQUIVALENT COMBINATION OF RELEVANT EDUCATION AND/OR EXPERIENCE Licensure/ Certification Requirements Must have current or compact RN licensure in the state of North Carolina. Basic life support (BLS) required. Advanced Cardiac Life Support (ACLS) certified Working Conditions Must be able to work under time pressures in a busy clinic/office environment. Must be present on site for 8 hour shifts during standard business hours. Must participate in group activities requiring interpersonal skills & cooperation. Must work beyond standard business hours according to business needs. Must be able to react quickly and immediately respond to emergencies. Must travel between locations on needed basis. Must be able to handle multiple assignments, conflicting demands & priorities. Must maintain attention to detail over extended period of time. Must be continually aware of variations in changing situations. Must be able to move throughout office environment throughout day. Mustbe able to lift minimumof 50 lbs. Must be able to push/pull minimum 100 lbs. with aid of a wheelchair Must be able to carryminimum 20 lbs. Must be able to reach for objects by extending arms.
