Job Description:
Position Summary This position will serve as a Case
Manager/Human Services Practitioner with the Encompass First
Episode Psychosis Program and OASIS program. Encompass and OASIS
aim to provide exceptional care and treatment to adolescents and
young adults from 15-30 years of age with recent onset psychosis,
or individuals at risk for psychosis. Encompass and OASIS are
coordinated specialty care clinics utilizing a team approach to
provide wrap-around services to a diverse group of clients from
various socioeconomic, cultural, and ethnic backgrounds. This
position will serve individuals and families in Wake, Orange,
Durham and surrounding counties. Some interventions may be
delivered virtually; however, the majority of the services will be
provided in the community, within the identified service areas.
This position will focus on the provision of direct case management
support for identified Encompass participants, including referrals
and linkage to community resources, assistance in navigating care
systems, and coordination of services. Case management seeks to
improve access to care, reduce barriers and improve outcomes for
individuals receiving services. Work Schedule Monday – Friday, 8:00
am – 5:00 pm; some evenings or weekends may be required.
