Join our Small House team and enjoy providing individualized care in a homelike setting! The Forest at Duke, a not-for-profit Continuing Care Retirement Community, is thrilled about the January 2023 opening of our newly-constructed, state of the art, spacious 90-bed Health Center. Introducing the Small House concept to our community, the new Health Center features four "neighborhoods," each with two households or "small houses" that support varying levels of care within our established continuum: Assisted Living, Memory Care, and Skilled Nursing.

While health centers can often feel fragmented and institutional, The Forest's new Small House model of care provides individualized care in a homelike setting. Supported by around-the-clock care from a collaborative team, residents will be empowered in their decision making, maintaining their autonomy regardless of their health challenges.

Certified Nursing Assistant (CNA) Care Partners are responsible for the delivery of holistic resident centered, resident-directed care that promotes resident autonomy, potential, and choice. The CNA Care Partner provides care regarding residents' ADLs and work in collaboration with the house care partners to manage the residents' environment, including cooking, laundry, and housekeeping.

Our Small House Model goal is for CNA Care Partners to have a 1:5 care ratio to allow them the opportunity to provide outstanding care.

Our CNA Care Partner rates are $14 - $16/hr. (determined based upon experience) plus these differentials:

  • Days: + $3 differential
  • Evenings: + $4 differential
  • Nights: + $5 differential All shifts available for full time and part time positions!

    Days: 7am-3:30pm, Evenings: 3pm-11:30pm, Nights: 11pm-7:30am

    Weekends only also available (same differentials apply)!

    Key Responsibilities:
    • Provides care functions as directed by Nurse Care Manager and outlined in the resident's care plan. Personal care may include shower/bath, shampoo, perineal care, oral hygiene, positioning, nails short and clean, dressing, range of motion, ambulation and insure adaptive devices are worn (i.e. dentures, hearing aids, glasses, etc.).
    • Performing other related duties as required by the resident.
    • Performs restorative and rehabilitative procedures per care plan.
    • Observing residents and reporting any reactions or changes in condition to the Nurse Care Manager.
    • Listening to and giving reports, and reviewing care plans daily.
    • Participates in the collaborative care planning meeting with the clinical support team and the resident. Key Requirements:
      • Active certification in good standing as CNA, with the right to practice in North Carolina.
      • Prior experience in long-term care (skilled/assisted) or CCRC is highly preferred.
      • Experience with Electronic Health Records needed, specific experience with Point Click Care is highly regarded.
      • Strong communication skills; able to communicate clearly and effectively.
      • Must show ability to read, write, speak, and understand English.
      • Must have certification in ServSafe - and BLS/CPR. Can be obtained during first 90 days of employment. The Forest provides:
