Job Description:
Duke Homecare & Hospice Duke Homecare & Hospice offers hospice,
home health and infusion services as well as serves as the home for
the Duke Caregiver Support Program. Our Hospice team works closely
with a patient's physician to provide comprehensive, individualized
care in the comfort their home or at our inpatient hospice facility
located in Durham. Our team of nurses, therapists, social workers,
counselors, and chaplains help patients and their families manage
advanced medical conditions. Duke Home Health provides services in
a patient's home that include skilled nursing, physical therapy,
occupational therapy, speech therapy, social service, home health
aides, and pain and wound management. Our infusion team comes to a
patient's home to administer intravenous medications, including
antibiotics and chemotherapy, to children and adults. Our nurses,
pharmacists, dietitians, and patient services coordinators work
under the direction of a patient's doctor to monitor needs and

8a-5p and 9a-6p with rotating back up every 4 weeks weekend
shift.
JOB SUMMARY
Coordinate internal and external referrals for DHCH and responsible
for initial intake of provider orders.
MAJOR JOB RESPONSIBILITIES
- Provides nursing care for patients in accordance with
physician's order and established policies and procedures
- Participates in weekly team meetings and case conferences, as
necessary, to facilitate that appropriate services are provided to
the patient
- Maintains up-to-date knowledge of Medicare/Medicaid/JC through
in-service and workshop attendance
- Coordinates care plan activities with multidisciplinary team
members as directed by Nursing Case Manager
- Documents care and services provided in accordance with DHCH
policies, procedures and regulatory standards
- Provides documentation that is timely, concise and
complete
- Follows standard precautions and uses Personal Protective
Equipment (PPE) as required
- Observes patients' reactions to medications and reports any
adverse reactions to the supervising nurse or physician
- Communicates regularly with team members, caregivers and
physicians
- Implements agency policy, procedures and objectives
- Maintains confidentiality of employee and patient
information
- Exhibits an attitude which promotes harmony and goodwill in the
workplace
- Displays a clean, neat and professional appearance
- Participates in Performance Improvement Program
- Promotes quality, comprehensive services through a team
approach.
- Performs other duties as assigned Education
Work requires graduation from an accredited Practical Nurse
program.
Experience
- One to two years of experience preferred
- Working knowledge of Medicare and Medicaid regulations.
- Strong Computer skills preferred Degrees, Licensure, and/or
Certification
- Current State of North Carolina LPN licensure is required.
- Valid NC driver's license
- AHA BLS Knowledge, Skills, and Abilities
- Knowledge of scope of licensed practical nurse, ability to
delegate to nursing support staff.
- Considerable knowledge of the care and treatment of patients
and special procedures that apply to practical nursing. Able to
independently seek out resources and work collaboratively.
- Able to communicate clearly with patients, families, visitors,
healthcare team, physicians, administrators, leadership and
others.
- Able to teach patients and families in accordance with the
nursing plan of care.
- Able to use sensory and cognitive functions to process and
prioritize information, treatment, and follow-up.
- Competent in BLS and/or other specialized life support
requirements designated by work area or organization.
- Able to use fine motor skills. Able to record activities,
document interventions.
- Able to use computer and learn new software programs.
- Able to navigate the entity to provide clinical care for
patients.
- Able to withstand prolonged standing and walking.
- Able to remain focused and organized.
- Working knowledge of sterile techniques and special procedures
that are applicable to work performed.
- Working knowledge of procedures and techniques involved in
administering routine and special treatments to patients.
- Working knowledge of sanitation, personal hygiene and basic
health and safety precautions applicable to work in a
hospital.
- Working knowledge of infection control procedures and safety
precautions Ability to understand and follow oral and written
instructions.
- Working knowledge and completion of appropriate JCAHO and other
