  • Relocation Assistance! ----The Service Access Nurse will manage patient intake process (including patient triage and admissions)for non-critical transfer patients into Duke Unive rsity Health Systemfrom referring hospitals, clinics, and physicians off ices, ensuringappropriate level of care and financial screening.----Work Performed--Evaluate appropriate level of patient care, u tilizing expert levelclinical decision making to determine severity of i llness for allelective, urgent and emergent admissions. Requires expertlevel analysisof patient clinical information and formulation of an accu rateassessment of the patient and his/her condition.Coordinate with pati ent, physicians, nursing and/or clinical staff whenrequirements of admis sion are not met/authorized, requiring facilitationof alternative care a rrangements, physician/administrative override, ordelay in service.Respo nsible for collecting accurate and viable patient insuranceinformation, and escalating all self-pay patients through theappropriate financial screening process (i.e. POMC, Medicaid, CharityCare, patient liability).Co ordinate with physician, nursing and/or clinical staff in determiningtha t the following have all been secured: (1) referral authorizations,(2) p rior approval of specific treatments and surgical procedures, and(3) sec ond surgical opinions of specific treatment/surgical procedures.Analyze clinical and financial information received on proposedadmissions for co ding accuracy, difficulty in obtaining authorization,and potential for financial issues .Initiate and facilitate pre-admission and pre-certifica tionauthorizations, requiring the detailed evaluation of clinicaldocumen tation.Responsible for ensuring all scheduled surgical cases are reconci ledwith the appropriate billing system.Ensure inpatient vs. outpatient s cheduling criteria are met inaccordance with published payer CPT lists.E nter detailed documentation of actions and outcomes in applicablebilling system.Health System resource for coding of ICD-9 diagnoses/procedures,CPT-4procedures, DRG, & APC assignment.Trainer and preceptor of Pre-Reg istration staff in ensuring theirclinical knowledge needed to fulfill th eir position responsibilities.Provide quality assurance reviews of Pre-R egistration staff work,ensuring internal controls are met.Conduct denial appeal follow-up, through analysis of clinicaldocumentation to support level of care provided and securereimbursement.Clinical and health care financial resource to PRMO and broader healthsystem membership.Expert in medical terminology and clinical data interpretation.Liaison to all hos pital and PDC departments and their staff members,including physicians, clinical nurses, Patient Resource Managers, clinicstaff and bed control.
