Job Description:
Duke Homecare & Hospice Duke Homecare & Hospice offers hospice,
home health and infusion services as well as serves as the home for
the Duke Caregiver Support Program. Our Hospice team works closely
with a patient-----TMs physician to provide comprehensive,
individualized care in the comfort their home or at our inpatient
hospice facility located in Durham. Our team of nurses, therapists,
social workers, counselors, and chaplains help patients and their
families manage advanced medical conditions. Duke Home Health
provides services in a patient-----TMs home that include skilled
nursing, physical therapy, occupational therapy, speech therapy,
social service, home health aides, and pain and wound management.
Our infusion team comes to a patient-----TMs home to administer
intravenous medications, including antibiotics and chemotherapy, to
children and adults. Our nurses, pharmacists, dietitians, and
patient services coordinators work under the direction of a
patient-----TMs doctor to monitor needs and treatment. Duke Nursing

Monday to Friday 8:30 AM-5:00 PM:: Every 8 weeks will be back up
for the weekend shift as needed. If the weekend shift worked, then
scheduled day off during the week.The DHCH Care Coordination team
consists of 12 Nursing Program Coordinators (NPC) working across
the 3 Duke hospitals. As a coordinator, you will have the
opportunity to work closely with the hospital team to include the
Case Manager, and Physician to initiate home care services for
those patients who need home health, hospice, or infusion services
at discharge. We provide education to the patient and their
caregiver related to the services needed to facilitate an
appropriate discharge plan. The NPC will utilize their strong
assessment skills and clinical knowledge in order to fulfill our
goal to provide the patient/family with a smooth transition to
home.JOB SUMMARY:
- Responsible for the coordination of home health, hospice, and
infusion services for the hospitalized patient, clinic/outpatient
and/or patient in an alternative setting (i.e. extended care
facilities) by acquiring admission information and assessing the
appropriateness of home care services.
- Responsible for providing community education, responding to
customer requests and concerns, and interfacing with referral
sources, organizations, government agencies and third-party
payors.
- Participate in care team unit huddles and function as an
educational resource.
- Provide consultation to physicians, hospitals, and other
referral sources regarding home coordination and available services
through DHCH.
- Meet with the referral source regularly to identify patient
population needs, referral process improvements and identify
patient/staff educational opportunities.MAJOR JOB FUNCTIONS:
- Maintains current knowledge base of the comprehensive services,
products and supplies available through DHCH.
- Maintains current knowledge of federal and state regulatory
guidelines related to home care services.
- Remains current of reimbursement issues, and maintains
understanding of their impact on the delivery of home health,
hospice, and infusion services.
- Functions as an educational resource, providing consultation to
physicians, hospitals, and other referral sources regarding home
care coordination and available services through DHCH.
- Utilize comprehensive assessment skills to determine patients
appropriateness of home care and initiate development of a plan of
care.
- Meet with referral sources regularly to identify patient
population needs, referral process improvements and identify
patient/staff educational opportunities.
- Maintains daily record of referral source/community contacts
and assists in identifying referral trends and needs.
- Initiates home care coordination in collaboration with referral
source, MD, patient/caregiver and central intake staff.
- Participate in patient case conferences. QUALIFICATIONS:To

Education, Degrees and Formal Training:BSN required. Current or
compact RN licensure in the state of North Carolina is required.
BLS required. Work Experience:Three years of nursing experience are
